Great Lakes Practice Transformation Network is pleased to welcome Dr. Nivedita Mohanty, MD, of AllianceChicago, for a webinar called, “The After Visit Summary, Not an After Thought.” Dr. Mohanty will discuss how clinicians can effectively use patient communication tools in the safety net setting, and will share her experience with modifying the After Visit Summary (AVS). She’ll go over lessons learned during the process, including the benefits of engaging with both patients and health care providers before and during the project. Dr. Mohanty will speak about the design features that enhanced effectiveness for a wide range of patients. She’ll also describe challenges her team faced and address how they overcame difficulties. Q&A will follow the presentation.
Learning Objectives:
1. Understand the utilization of patient facing communication tools such as the After Visit Summary (AVS) amongst clinicians in safety net primary care clinics
2. Explore feedback on design features for enhanced usability of clinical information on the AVS for the benefit of diverse populations.
3. Discuss the benefits of engaging clinical care team members and patients in actual clinical settings to inform the design of patient-facing tools
